Medical
Information
In order to provide a safe environment for all campers, Camp
Stella Maris requires a full disclosure of significant medical and behavioral
information for children wishing to attend. Once registered, completed health
information is required.
General
Information
Age
Requirement:
To attend Resident Camp, campers must be 15 or younger during
their entire session. Please select sessions prior to the camper’s 16th
birthday. 15 year-olds are eligible to attend Resident Camp prior to attending
LIT session.
Bring A
Friend:
If you are a returning or new camper who is already registered for
the 2008 summer program, “bring-a-friend” who has never participated in the
summer program, and receive a $10 gift certificate to the CSM store. Only one
referring camper will receive reward (no reward if multiple names listed) of
gift certificate upon Sunday check-in. If referring camper cancels gift
certificate will be forfeited.
Camp Store:
Each Resident Camper, LIT and CIT is allotted $6 per week ($4 for
Day Campers) from their registration fee to spend in the camp store. No
additional money may be added. Each camper is limited to one visit per day.
There are no refunds. If a camper does not use the entire allotment by the end
of the week, the remaining balance is transferred to the CSM Campership Fund.
Camper Code
of Conduct:
In order for a camper’s time at CSM to be safe and enjoyable, we
have set forth certain expectations. These expectations will be introduced and
reviewed with each camper upon arrival and we require that each camper live by
this Code of Conduct while attending Camp Stella Maris.
CIT
Procedure:
All CIT registration forms must be postmarked by Saturday,
November 26, 2007, to be entered into a drawing to determine which campers will
be registered in the program. Mail notifications will be sent to all candidates
on Friday, December 7, 2007.
Financial
Assistance:
Need-based assistance is available. Call 585-346-2243 ext 105 or

to request an application.
Full Payment:
Must be received by April 1, 2008. A $15 fee is charged for all
checks returned due to insufficient funds. For your convenience, CSM offers
multiple payment options for balance dues (see registration form).
Hold-Over
Weekends:
AB, CD, and GH include a Saturday evening stay. No Saturday
evening stay between E & F week. There will be a field trip to Minnehan's,
a nearby recreational facility.
Late Fee:
The completed CSM Health Form and payment due must be postmarked
by April 1, 2007, or a $25 processing fee per child will be assessed.
LIT Procedure:
All LIT registration forms must be accompanied by an LIT
Application. The application can be downloaded from our website or mailed by
contacting the camp office. These forms must be postmarked by Sunday, December
31, 2007. Mail notifications will be sent to all candidates on Friday, February
1, 2008.
Medical Forms:
The completed Camper Health Evaluation Form must be received by
April 1, 2008.

Open
Registration:
Prospective campers and families are invited to visit CSM and
register for our resident and day camp programs. Come visit and tour CSM on
Saturday, April 12, 2008, from 9 a.m. to noon.
Open House:
All are invited to attend an open house Sunday, June 1, 2008, from
noon to 3 p.m. Those attending will be able to tour the resident and day camp
facilities and meet staff members.

Rates for this program are as follows: Session A:

$75: not attending additional camps during Summer at CSM

$70: attending additional camps during Summer at CSM
Sesssion B:

$105: not attending additional camps during Summer at CSM

$95: attending additional camps during Summer at CSM
Refunds:
If cancellation is made 10 days prior to the start of your program
session, a full refund, less the deposit, will be issued. If cancellation is
made less than 10 days prior to the start of your program session, a 50%
refund, less the deposit, will be issued. In case of injury or illness, if the
registration office is notified in writing by the attending physician, a full
refund, less the deposit, will be issued. Once a camper arrives at camp, no
refund will be issued.
Registration
and Fees:
Starting November 1, mail or fax registration forms to: Camp Stella
Maris, 4395 East Lake Road, Livonia, N.Y. 14487 or (585) 346-6921. Be
sure to clearly check the week(s) desired and sign each form (one per child).
Upon receipt of registration, confirmation packet will be e-mailed or mailed.
Beginning Nov. 14, registration will also be accepted by phone (585)346-2243,
ext. 104 or walk-in. To be processed, forms must be accompanied by a
$75 non-refundable deposit.
Session
Limit:
Resident campers are allowed to register for a limit of four weeks
per summer. This limit does not apply to day campers.
Sibling
Discount:
Receive a $20 discount per camper per session on each additional
child registered per household. Discount is redeemable for summer programs only
during the same season. If two siblings registered and one cancels, discount
will be forfeited.
Transportation/Arrival
and Departure:
Transportation must be provided by parents. For resident camp the
arrival is Sunday from 3-5 p.m. Departure for weeks A, C, E and G is Saturday
from 9-11a.m. Departure for weeks B, D, F and H is Friday from 5-6 p.m. For day
camp, early drop-off begins at 7:30 a.m. (see registration form). Regular
drop-off is 8:45-9 a.m. and departure from 5-5:30 p.m.
Visiting:
Parents may visit their children on Saturday from 9 a.m. - 2 p.m.
and Sunday from 10 a.m. - 4 p.m. Visits during the week are strongly
discouraged. All visitors must stop at the infirmary or the camp office for
security pass. A security pass is required for all visitors.
